More Than a Provider. A More Complete Approach to Care.

Seventeen years of experience. Dual board certifications. One mission: helping you feel like yourself again.

The Pattern Nobody Was Talking About

Early in my career, I started noticing something that bothered me.

Hi, I’m Tremone McLeod, FNP-BC, PMHNP-BC,

I built this practice because patients would come in struggling, truly struggling, and they'd already seen two, three, sometimes four different providers. A therapist for the anxiety. A physician for the fatigue. A specialist for the sleep. Everyone treating their piece of the puzzle, and nobody looking at the whole picture.

And the patients? They were exhausted. Not just from what they were going through, but from having to explain themselves over and over again — to people who never quite communicated with each other.

The anxiety was making the insomnia worse. The insomnia was fueling the depression. The depression was showing up in their bloodwork, their weight, their energy levels. These things were connected. They had always been connected.

I just couldn't figure out why we kept treating them like they weren't.

Rhys-Lan is named after my two children — Rhys and M'Lan

The way I've woven their names together is exactly how I approach care: two things that belong together, unified into something stronger than either one alone.

Mental health and physical health aren't opposing forces. They're two sides of the same person. And for too long, the healthcare system has treated them like separate departments — separate referrals, separate waiting rooms, separate conversations that never happen.

I built Rhys-Lan Integrative Health to close that gap. To be the practice I kept wishing my patients already had access to. One where you don't have to choose between your mind and your body, because we take care of both — together, from the very first appointment.

This practice exists because you deserve care that sees all of you.

The Qualifications Behind the Care

17+ Years of Clinical Experience

Over seventeen years working directly with patients across primary care and behavioral health settings — not in theory, in practice.

Double Board-Certified

Board-certified in both primary care and psychiatric-mental health as a nurse practitioner — a rare combination that makes integrated care possible under one roof.

Evidence-Based, Always

Every treatment recommendation is grounded in current, peer-reviewed clinical guidelines. You'll never receive care based on trends or guesswork.

Integrated Care Expertise

Trained and experienced in treating the intersection of mental and physical health — including anxiety, depression, addiction, insomnia, and chronic condition management.

What You Can Expect Here

This isn't a fifteen-minute appointment where you leave with a prescription and more questions than answers.

You'll be heard. Not just your chief complaint, your full story. Your history, your patterns, what you've already tried, what hasn't worked, and what you're hoping for. We start there.

You'll be understood. Your mental and physical health will be looked at together, the way they actually exist in your body, not split into separate boxes that nobody connects.

You'll leave with a plan. Not vague next steps. A real, personalized, evidence-based plan built around your specific needs, one that evolves as you do.

You'll never feel rushed. Because getting care right the first time is always faster than starting over.

If You're Ready For Care That Actually Connects The Dots, You're In The Right Place.
